Please come to the surgery to collect the necessary forms for registration with the practice. You can also begin the registration process online (see below). Once accepted by the practice, registration details are passed to the Health Authority by way of a computerised link.
NHS England has determined that all patients be assigned a named GP who is responsible for patients' overall care at the practice. If you wish to know who your named GP is please contact the practice. If you have a preference as to who that GP is, the practice will make reasonable efforts to accommodate a request.
We are required to ensure that every patient has access to a medical practitioner within 48 hours of a request being made. Requests to see a particular Doctor, however, might mean having to wait longer for an appointment.
If you wish to pre-register, click on the link below to open the form. When you have completed all the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time, you will be asked to sign the form to confirm that the details are correct.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it with you on your first visit to the practice.
