Our online appointment booking service is currently suspended

GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:

  • book or cancel appointments online
  • renew or order repeat prescriptions online
  • view your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results

You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. How to sign up for online services

For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you don’t have these, please talk to us and we will help you sign-up

If you are a new patient you will be given an information leaflet and consent form to complete at the time of registration for you to be given access.

If you are an existing patient, then come in to the Practice and let the receptionist know you would like to register for our Online Services.  Once the Reception staff have verified your photo ID and proof of residence against your patient record, you will be given a unique username and password along with details of how to gain access to the website.  If you cannot produce photo ID, the Practice Manager will use their discretion regarding what document(s) may be submitted instead on an individual basis.

Rudgwick Medical Practice will accept applications from patients as well as their proxy. Proxy access refers to access to online services by somebody acting on behalf of the patient with the patient’s consent. The appropriate application form must be completed prior to any online access being enabled.

If you are an existing patient and would like to request additional access to your detailed coded record, you will need to come into the surgery with ID and complete the detailed records consent form requesting this. This is then passed to a clinical member of staff to check and can take up to 15 working days to process.

Please note that all forms that require identity verification must be completed at the surgery in the presence of the receptionist.
